Milrinone is a newer cyclic AMP specific phosphodiesterase (PDE) inhibitor (also called Fraction III or low-Km cyclic AMP) that can produce both positive inotropic
effects and vasodilation independent of ß1-adrenergic receptor stimulation in the
cardiovascular system.  Milrinone improves hemodynamics and biventricular function in patients with ventricular dysfunction by increasing stroke volume index, increasing left ventricular contractility, producing pulmonary vasodilation.  Milrinone also produces vasodilation in arteriolar and venous vascular smooth muscle.  Compared to amrinone, another bipyridine derivative, milrinone is a more potent drug with a much shorter context sensitive half time (duration of action) and fewer side effects.
